Lost radios will be regrouped to the Reward Talk group. Stolen radios will be inhibited.

I am an authorized Agency representative and I approve all requests made on this Form. Bring this form with the radio(s) to the RCS Programming Shop at 5595 Overland Ave., Suite 101, San Diego CA 92123 to be deprogrammed. We will notify you when the radios are ready for pickup.

Please bring in original packaging for all new mobile radios.
